Reminders (for visits)

- Bring your insurance card to every

full is required prior to your visit.

Know Your Contract
Your health insurance is a valuable asset; you should know
and understand completely the terms of your contract. Because of the number of
insurance companies and plans available, we cannot keep track of individual
plan definitions and requirements (pre-authorizations, pre-certifications,
co-pays, waiting periods, and so on). The more you know about your own plan,
the greater the benefit you can enjoy. Please bring your current insurance card
with you on each visit to the clinic.
Contact Your Company
If you have questions about your insurance coverage or wish
to raise a concern about the manner in which your insurance company paid our
bill, please call your insurance company directly. They need your input in
order to improve their service.
Filing a Claim
As a courtesy, we will file your claim to your primary
insurance company one time for you. We also submit claims to secondary
